Horizon Homes, Inc. Corporate Adult Foster Care and Board and Lodge with Special Services programs are seeking direct care staff to work with individuals with mental illness in residential settings. Interested applicants must have a minimum of a high school diploma, be at least 18 years of age, have a valid driver's license with proof of insurance, and pass a background study. Experience is preferred.

Mobile Crisis Team Mental Health Practitioners

Job Description:

Horizon Homes, Inc has immediate openings for full-time and part-time mental health practitioners for our mobile crisis team and South Central Crisis Center. The mobile crisis team serves individuals and families in the 10 county area of the South Central Community Based Initiative (Blue Earth, Brown, Faribault, Freeborn, Le Sueur, Martin, Nicollet, Rice, Sibley, Watonwan). It operates 24 hours a day, 7 days/week. Mental health practitioners are in-office during these hours and respond to calls in the community. They are supervised telephonically by a mental health professional.

While on-scene, the mental health practitioner will complete mental health assessments and provide intervention skills to assist clients in working through a crisis situation. This team serves both children and adults, and individuals with experience working with children are strongly encouraged to apply.

Mental Health Practitioner

Horizon Homes, Inc. Intensive Residential Treatment Facility has a part-time opening for a Mental Health Practitioner.  Hours include every-other weekend and pickup shifts as needed.  Employees work in a team setting, assisting with clients’ needs and addressing any situations that may arise. Services provided may include crisis intervention, providing individual education related to mental health, fostering a safe environment, and communicating with the on-call Mental Health Professional as needed.
Preferred qualifications include:

  1. Must have 6000 hours of supervised mental health experience, or;
  2. Must have a bachelor’s degree in a behavioral science and 2000 hours of supervised mental health experience, or;
  3. Must have a master’s degree in mental health
